We asked Durado Brooks, MD, MPH, director of prostate and colorectal cancers for a response to the study.

??The ?gender gap? in polyp prevalence described in this study has been demonstrated by other researchers. The current study is an important addition to this literature due both to the large size of the study population and because most or all study participants underwent screening (as opposed to diagnostic) colonoscopy.

?However, the applicability of these findings to the U.S. is somewhat limited, as the Austrian population is much more homogenous than the population of the United States. This is an important issue because there is a wealth of data demonstrating differences in the prevalence of both polyps and cancers between various racial and ethnic groups in the U.S. In particular, African Americans have an earlier age of onset and strikingly higher rates of colorectal cancer compared with other groups, as well as an increased prevalence of large polyps. The type of gender differences in polyp prevalence shown in the Austrian study have not been documented for African Americans.

?It is also important to note that, while polyps and polyp prevalence are an important factor in determining the appropriate age to initiate colorectal cancer screening, the incidence of colorectal cancer cases and deaths is of greater import. There is a significant increase in colorectal cancer incidence and deaths beginning at age 50 for both men and women in the U.S. Indeed, the incidence rate for men and women between the ages of 50-54 are twice those of individuals aged 45-49, and colorectal mortality nearly doubles in that interval as well. Based on this data, and the substantial decrease in colorectal cancer incidence and mortality attained over the past 2 decades, experts have agreed that 50 remains a reasonable starting age for men and women at average risk.

?U.S. women continue to lag behind men in colorectal cancer screening, and one of the biggest obstacles to increasing the use of well-proven lifesaving colon cancer screening tests is the belief that colorectal cancer is a ?man?s disease.? Unfortunately, this study could inadvertently feed into that belief. The Society will continue to reinforce the fact that colorectal cancer is the third leading cause of cancer death in both men and women, and not a ?man?s disease. The fact that advanced adenomas and cancers are somewhat more common in men is widely known. However the numbers in women are still very significant, a point it is important to continue to emphasize.

?The idea of different screening recommendations based on individual factors (gender, age, race/ethnicity) has been explored during each review of the Society?s colorectal cancer guidelines. This study notwithstanding, there is limited evidence demonstrating that such customized recommendations will improve screening and outcomes. At the same time, there are concerns that such variable guidance could lead to confusion among patients and clinicians and paradoxically worsen colorectal cancer screening rates for some groups. This issue will be thoroughly evaluated in our future guidelines development activities.?

It must be noted that most prostate tumors are slow growing and many patients live long and die of other causes or diseases; the tumor may even be found only at autopsy.

Diagnosis of Prostate Cancer

Cancer of the prostate may be initially detected by routine physical examination when the doctor does a digital rectal examination. The doctor will feel an enlarged prostate that is firm or hard. Sometimes it may also be initially detected through routine blood screening for prostate-specific antigen (PSA).

Since signs and symptoms and even lab exams may resemble other conditions of the prostate such as benign prostatic hyperplasia (BPH), prostate cancer can be diagnosed definitively by doing a needle biopsy.

To find out if other organs have been involved by the possible spread of cancer, CT scans and bone scans may be done.

Treatment of Prostate Cancer

There are many treatment options for malignant neoplasms of the prostate. Since it is a slow growing tumor, if it is found in the elderly, the doctor may opt to just observe the patient and monitor PSA levels instead of doing any aggressive measures.

In younger patients treatment will depend on the stage of the disease. Those in early stages have the following options:

Complications of these therapeutic modalities include impotence, difficulty in controlling urination and bowel problems.

Prognosis of treatment depends on the stage of diagnosis and the general health of the patient.

LG Optimus Hub (aka Univa / Optimus Two) earns its stripes at the FCC

Okay, we'll totally admit that we've got something of a phone crush on LG's successor to the Optimus One lineup. Sure, it hasn't debuted in the States, and its specs already look a bit tired, but there's tons of goodwill attached to this family of devices. Should LG properly execute, the handset promises to be an iterative improvement for all smartphone buyers that need to save some coin. Now this sexy creation is making its way though the FCC, and while it's difficult to tell whether this is an international model or is bound for our domestic shores, it's easy to realize that we're one step closer to its much anticipated release. After a bit of digging, we can tell you the phone supports the 1900 and 850MHz bands, which is common to networks such as AT&T, Bell, Telus and Rogers. With all the names being tossed around, it'll be interesting to see how the individual carriers choose to brand these devices, but it seems that we won't have to wait much longer for those details.

'Shore's' Snooki gets a 'ciao' from Jionni

How dare MTV leave us hanging once Snooki crashed into a police car! Was she going back to jail? Were they going to get kicked out of Italy? Did that really happen?

Yes, it did happen, but luckily we found out in the beginning of tonight's episode that our little meatball and Deena avoided time behind bars and the "Jersey Shore" kids are staying in Florence. But Snooks isn't allowed to drive anymore. That's probably best.

Anyway, all of that is minor compared to the drama that came later...

This was the night when viewers finally got to see that Snooki's boyfriend, Jionni, was on his way to Italy to see his lady. Eek!

Unfortunately, JWoww's honey, Roger, couldn't get time off work to visit. Jenni's heart was broken, and it broke the other girls' hearts (and ours) to see her so disappointed. So they went out, while the guys had an MVP night ? plus Ronnie ? to try and find some ladies that were DTF.

Snooki stayed at home by herself until she got a visitor.

Surprise! It's Brittany, Mike's slightly stalkerish admirer who has a twin sister that likes to make out with practically everyone in the house.

In an effort to pull the ultimate prank, Snooks let the blond in and had her hide in Mike's bed so he can get caught bringing another girl home. And that's exactly what happened....

Too bad it backfired, because in the end, Snooki only helped him hook up.

The next day was Jionni's arrival. OMG, so exciting! There was no time to introduce him to everyone because the miniature guidette was ready to smush, so they got to knockin' da boots, and Snooks exclaimed how she can't wait to have his guido babies ? but she'll get a C-section so it doesn't "(mess) up (her) vagina."

Meanwhile, Mike kept acting like he was the next Karate Kid and keeping an eye on Jionni. He wasn't bitter. Or crazy. Nope, not one bit.

Snooks and her man were pretty much adorable and super in love, then the alcohol took over her body and brought out her not-so-ladylike behavior.

Remember, Jionni gets embarrassed easily, so when he saw his girlfriend showing her kooka to everyone, he was mad and stormed out.

Jenni tried to ease the situation, but Snooki got into a yelling match with her instead and said some pretty hurtful things. Regardless, JWoww trucked it all over Florence to try and find her friend's man. Um, Best Friend of the Year Award?

Snooki doesn't stop crying for the remainder of the episode which was sad but also slightly annoying.

Jionni was still fired up when he got back to the house and barely acknowledged his girl, only to tell her she wasn't his girl anymore, and then got his stuff and left.

Basically, the guy traveled over 20 hours to see his significant other in Italy, but only spent about six hours there before leaving a single man.

Some trip, eh?

Breast milk antibody fights HIV but needs boost, study finds

ScienceDaily (Sep. 19, 2011) ? Breast milk antibody both neutralizes human immunodeficiency virus (HIV) and kills HIV-infected cells, according to a paper in the September 2011 issue of the Journal of Virology.

"This finding indicates that enhancement of these responses through vaccination could help reduce HIV transmission via breastfeeding," says corresponding author Sallie Permar of Duke University, Durham, NC. While HIV-specific antibodies have been identified in breast milk, this is the first study to investigate the virus-blocking functions of these antibodies.

Nonetheless, the statistics indicate that breast milk antibodies are doing an incomplete job of protecting babies from HIV transmission. Nearly half of the 350,000 new infant HIV infections occurring annually are transmitted via breast milk. Permar's study provides perspective: it shows that the magnitude of anti-HIV responses in breast milk is low compared to those in plasma.

Thus, the need to enhance that response. Fortunately, the study's results suggest that current systemic HIV vaccine candidates may be effective in enhancing anti-HIV functions of breast milk antibody, and reducing postnatal HIV transmission. Permar et al. found that breast milk antibody's activity against HIV and HIV-infected cells is mediated by IgG antibodies that originate in the blood stream, rather than IgA antibodies, which are produced in the mammary gland.

The alternative to immunologic intervention, formula feeding, "is not a viable option for reducing this mode of transmission in resource poor areas with high HIV prevalence, as it is associated with high infant mortality from diarrhea and respiratory illnesses," says Permar. "While maternal and/or infant antiretroviral prophylaxis during the period of breastfeeding is effective in reducing infant transmission, HIV transmission continues to occur in the setting of optimal prophylaxis and the effects of this long-term prophylaxis on infant growth and development are not known. Moreover, long-term prophylaxis is a challenge for resource poor areas." But a maternal or infant vaccine would be ideal for eliminating post-natal HIV transmission, she says.

Southern Calif grocery workers pave way for strike

LOS ANGELES (AP) ? Grocery workers could go on strike as soon as Sunday night in response to inaction on health care benefits, the workers' union said Thursday.

Eight months into contract negotiations, Southern California grocery workers issued a 72-hour notice Thursday night to cancel a contract extension and pave the way for a strike, said Rick Icaza, president of Grocery Workers Union Local 770.

A strike isn't guaranteed for workers at Albertsons, Ralphs and Vons, but cancelling the contract removes the final barrier to a strike.

In separate statements Thursday, all three grocers said they were disappointed in the union's move and they will remain in active negotiations.

"We don't want a strike, and we hope to continue bargaining rather than continue to alarm our associates and our customers," said Albertsons spokeswoman Christie Ly.

Ralphs spokeswoman Kendra Doyel said her company remains committed to reaching an agreement.

Vons spokesman Brian Dowling urged the union to remain focused on the negotiation process, while pledging to do the same.

Contract negotiations are stalled because supermarkets are unwilling to compromise on health care contributions for 62,000 grocery workers, Icaza said.

Ralphs spokeswoman Kendra Doyel has said her company currently pays more than 90 percent of health coverage costs. Ralphs workers hired before 2004 pay nothing toward their premiums and those hired later pay either $7 a week for single coverage or $15 a week for family coverage.

A four-month strike and lockout that began in 2003 cost Ralphs and other grocery chains an estimated $2 billion.

A wild and woolly discovery: Tibetan expedition ends with prehistoric find

Yang Wang is known for conducting complex research using highly sophisticated equipment. Yet the Florida State University geochemist also has spent days hiking through the remote outback of Tibet and camping in the foothills of the Himalayas ? all in the name of scientific discovery.

Because of that unique mix of skills, Wang was chosen to take part in a team of researchers that uncovered the oldest prehistoric woolly rhino ever found. A paper describing the team's discovery was just published in Science (http://www.sciencemag.org/), a prestigious journal established in 1880 by Thomas Edison. Wang and an international group of paleontologists set out in 2007 to explore one of the most isolated places on earth: the Zanda (ZAH-dah) Basin in Tibet, located at the feet of the Himalaya Mountains. The words majestic, wild and awesome all apply, yet fail to capture the landscape's natural wonder.

What drew the researchers to the basin wasn't its raw beauty, however. They came to explore its buried treasures. The largely untouched Zanda Basin is a fossil hunter's paradise, and the team was determined to make scientific breakthroughs.

They did just that, finding the complete skull and lower jaw of a previously unknown and long-extinct animal. They christened it the Tibetan woolly rhino (Coelodonta thibetana).

"This is the oldest, most primitive woolly rhino every found," Wang said of the team's discovery.

The ancient beast stood perhaps 6 feet tall and 12 to 14 feet long. It bore two great horns. One grew from the tip of its nose and was about 3 feet long. A much smaller horn arose from between its eyes. The Tibetan woolly rhino was stocky like today's rhino but had long, thick hair. It is often mentioned in the same breath with woolly mammoths, giant sloths and sabertooth cats, all giant mammals of the period that became extinct.

Prior to the team's discovery, the oldest woolly rhino ever found was 2.6 million years old, making it an inhabitant of the Pleistocene era (2.6 million years ago to 11,700 years ago). But the Tibetan woolly rhino found by the team is 3.7 million years old. That means it lived during the Pliocene epoch (5.3 million to 2.6 million years ago).

The new time frame also indicates that the Tibetan woolly rhino was alive before the last Ice Age. Wang examined the chemistry of the rhino's fossilized teeth using a special instrument, a mass spectrometer, at the National High Magnetic Field Laboratory (http://www.magnet.fsu.edu/) at Florida State.

"We look at the chemistry of the teeth and bones, to see what the animals ate and what kind of environment they lived in," said Wang, a professor in the university's Department of Earth, Ocean and Atmospheric Science (http://eoas.fsu.edu/).

Her detailed analysis revealed that the creature ate grasses that grew at high altitudes. That suggests, Wang said, that when the Ice Age arrived, the Tibetan woolly rhino adapted by moving from the mountains to lower altitudes.

The expedition team also found horse, elephant and deer fossils. Most of the fossils, including the Tibetan woolly rhino's complete skull, are being kept at the Chinese Academy of Sciences in Beijing, at its Institute of Vertebrate Paleontology and Paleoanthropology.

Wang and other members of the team, led by Xiaoming Wang, curator of the Natural History Museum of Los Angeles, plan to return to the basin again in the summer of 2012.

"Cold places, such as Tibet, the Arctic and the Antarctic, are where the most unexpected discoveries will be made in the future ? these are the remaining frontiers that are still largely unexplored," said Xiaoming Wang.
